Job Description:

Take on a critical-need travel healthcare contract as a Case Manager in Anaheim, California. You will coordinate services, monitor progress, support transitions of care, and help patients navigate the next steps in their healthcare journey. This assignment offers a structured weekday schedule and the opportunity to experience Southern California between shifts.
