Job Description:

Join a dedicated inpatient care team as a Case Manager focused on complex adult and geriatric populations in an underserved community hospital. During this 13-week contract, you will manage an average caseload of 14-20 patients across various units, including PCU, MedSurg, and LDRP, with occasional float to other inpatient areas. Your day will involve collaborating with multidisciplinary teams and physicians, navigating insurance plans, and assessing patient needs to facilitate resource allocation and discharge planning. This role offers the unique experience of working with a diverse, often challenging patient base, including unhoused and low-income individuals, in a facility that values critical thinking and teamwork. Expect to be engaged in facility initiatives, utilizing tools such as EPIC and Careport, and to contribute meaningfully to patient progression while maintaining a professional appearance in eggplant scrubs or business casual attire.

Job Specifics:

Location: Chicago, Illinois
Shift: 8-hour days, Monday through Friday, 0800-1630, with occasional longer days and weekend rotations every 5th weekend
