Job Description:

You will be the navigator on a ship, steering patient care through comprehensive assessments, care planning, and discharge coordination. Your day will involve prioritizing cases, collaborating with social workers and utilization managers, and participating in critical meetings like CAPP and CCM to ensure smooth patient transitions and optimal outcomes.
