Job Description:

This discharge planning role offers a rewarding opportunity to work closely with inpatient teams, social workers, and physicians on a Med-Surg or Tele unit. You’ll be responsible for coordinating patient discharges, making referrals, and ensuring patients transition smoothly to their next destination, whether that’s home, a community resource, or a skilled nursing facility. The assignment involves handling a caseload of 17-25 patients on most floors, with the chance to work independently while collaborating with an interdisciplinary team. This role combines the routine of discharge planning with the challenge of critical thinking—be prepared to navigate complex cases and juggle diverse responsibilities. Wearing business casual or neutral-colored scrubs is encouraged, and the work environment values professionalism with a touch of teamwork-driven playfulness. Expect to utilize your knowledge of discharge procedures, insurance approvals, and community resources to make a real difference in patient success.
