Job Description:

Join us as an RN Case Manager at Central Maine Healthcare in Lewiston, ME. This role involves coordinating discharge planning, managing patient cases, and collaborating with the healthcare team to ensure smooth transitions for geriatric, adult, and pediatric patients. You'll work on a stable day shift from 8:00 AM to 4:30 PM, providing 40 hours per week for a full 12-week contract.
