Job Description:

Join a healthcare team as a Case Manager Registered Nurse supporting complex cases across the system and the community, mostly through virtual interactions. You'll be coordinating patient care, navigating system resources, and floating between the Emeryville and San Francisco offices as needed. This role offers an engaging mix of community outreach and system-wide collaboration, all while working in a primarily virtual capacity. Your expertise in inpatient case management and California licensure will be key to success in this position, which features consistent weekdays and the opportunity to make a meaningful impact remotely.
