Job Description:

Join our team as a highly experienced inpatient case management professional at a busy Level 1 trauma hospital. This contract role involves managing acute inpatient cases across all hospital units, coordinating discharge planning, and working closely with a dedicated Social Work team. You'll be expected to adapt quickly, as minimal training is provided, and handle a patient load of 15-20 cases daily. The position offers a unique opportunity to work in a fast-paced environment with complex patients, including those requiring specialized placements at contracted facilities. Expect a professional yet engaging experience in a hospital setting that values efficiency and expertise, all while enjoying a supportive team atmosphere.
