Job Description:

Join a dedicated case management team at a small community hospital in Yakima, Washington, where you'll play a vital role in coordinating patient discharges and ensuring smooth transitions to appropriate next steps. This 13-week travel assignment involves managing a caseload of 15-20 patients daily in diverse settings including ER, ICU, MedSurg, NICU, Oncology, and TELE. Your expertise in discharge planning, assessment skills, and experience with patients facing complex social circumstances make you an asset here. The role features 4x10-hour shifts from 7:00 AM to 5:30 PM, with every other weekend rotation. While floating across the hospital is part of the job, you'll find your work fulfilling in this small but impactful environment, serving a large rural population with a trauma designation. Expect to utilize EPIC EMR, wear solid scrubs or business casual attire, and enjoy a balanced work schedule that keeps the focus on quality patient care coordination.
