Job Description:

Join us as a Case Manager Registered Nurse in Milford, MA, where you'll be stepping into a pivotal role on an ortho unit. This assignment is perfect for a seasoned RN with 3-5 years of acute care experience and a BSN certification. You'll coordinate patient discharges, facilitate care planning, and document using Epic, all while working daytime hours from 8:00 to 16:30. Every third weekend, you'll contribute your expertise to ensure smooth patient transitions, making a real difference in the hospital's patient flow. This 10-week contract offers a balanced schedule with 40 hours per week—an excellent opportunity to refine your case management skills in a vibrant setting.
