Job Description:

This case management RN position offers a unique opportunity to work within an acute care setting, overseeing patient pathways and coordinating care plans during regular daytime hours. The role involves assessing patient needs, collaborating with healthcare teams, and ensuring smooth transitions through the continuum of care. Ideal for a nurse with at least three years of acute care case management experience and a valid California RN license, you’ll use your expertise to make a tangible difference, all while enjoying the steady 8-hour shifts. Note: Epic system familiarity is highly preferred, so you can hit the ground running in this fast-paced role that’s as rewarding as it is challenging.
