Job Description:

Join us for a rewarding 13-week assignment as a case management or utilization review professional in Peoria, Illinois. In this role, you'll be part of a dedicated team working within an inpatient physical rehab or long-term acute care (LTAC) setting. Your days will involve coordinating care, developing individual plans for complex patients—including those with respiratory issues, wounds, or chronic illnesses—and collaborating with multidisciplinary teams to ensure smooth patient transitions from admission through discharge. This isn’t just a job; it’s an opportunity to make a real impact on patients navigating difficult health journeys. The work will be engaging, with a manageable caseload of 13-25 patients per day, and plenty of teamwork on interdisciplinary rounding and family meetings. The schedule offers consistent hours from 8:00 am to 4:30 pm, Monday through Friday, with no holiday work required.
