Job Description:

Join us for an engaging 13-week contract in Santa Rosa, CA, as a Case Manager Registered Nurse. This role involves coordinating utilization management, discharge planning, and post-acute care referrals for high-risk patients, especially those presenting to the Emergency Department. You'll work daytime hours from 8:00 am to 4:30 pm, with a schedule of five days a week, including at least two weekend shifts.
